The Norwood scale, stage by stage

The calculator’s first real question is your Norwood stage, so here is the whole scale in one picture. It is a top-down view of the scalp, front of the head at the top. The solid tone is hair that is still there.

How the estimate is calculated

The calculator starts from the graft ranges published for each Norwood stage in hair restoration planning literature:

  • Norwood 2 — 800 to 1,500 grafts
  • Norwood 3 — 1,500 to 2,200 grafts
  • Norwood 3 vertex — 2,000 to 2,800 grafts
  • Norwood 4 — 2,200 to 3,200 grafts
  • Norwood 5 — 3,000 to 4,000 grafts
  • Norwood 6 — 4,000 to 5,500 grafts, normally across two sessions

Each published range assumes the coverage that is typical at that stage: hairline only at Norwood 2 and 3, hairline plus mid-scalp at Norwood 3 vertex and 4, and the full scalp at Norwood 5 and 6. The calculator then scales the range by how the zones you actually select compare with that typical footprint, weighting the hairline and temples at 40 per cent of a full scalp, the mid-scalp at 32 per cent and the crown at 38 per cent, and capping any increase at 1.35 times the published range so the number cannot run away. Two further multipliers are applied. A thin donor area reduces the safe yield by about 15 per cent; a dense one raises it by about 6 per cent. Fine hair needs roughly 15 per cent more grafts to reach the same visual density, while coarse hair needs about 12 per cent fewer, because perceived density depends on total hair shaft cross-section rather than follicle count.

The hair count shown beneath the graft range uses an average of 2.2 hairs per graft, which is typical of a mixed harvest of singles, doubles and triples.

What the calculator cannot tell you

The calculator cannot see your donor area, and the donor area is the real constraint in almost every case. A patient at Norwood 5 with a thin, low-density donor zone cannot safely receive 4,000 grafts, and a surgeon who agrees to take them is borrowing from the back of the head to decorate the front. It also cannot assess active loss: transplanting into an area that is still shedding produces a result that looks thinner a year later, which is why medical stabilisation usually comes before surgery.
